Abstract

  Background and Objective: anemia is one of the main problems of chronic renal failure patients undergone Hemodialysis and erythropoietin is not effective for these patients. Since Vitamin C deficiency in Hemodialysis patients is common, this study was performed to compare the effect of intravenous and oral ascorbic acid consumption on anemia in Hemodialysis patients.

  Material and Methods: this clinical trial was conducted on 75 patients, selected via convenience sampling and including criteria (hemoglobin, 11mg/dl ferritin, 100ug/L transferring saturation, < 20%). The subjects were randomly assigned to three equal groups of control , oral ascorbic acid (POAA) and intravenous ascorbic acid (IVAA) After each every-other-day dialysis performed for eight weeks, the patients of IVAA group were given 300mg intravenous vitamin C and the ones of POAA group given 500mg vitamin C orally. The Plasma iron with chemical method, total iron-binding capacity (TIBC) and ferritin level with indirect method, hemoglobin with cell counter and transferring saturation (TSAT) with related formula were measured. Using a checklist, the data of demographic, laboratory results and side effects were recorded and analyzed by SPSS-13, using ANOVA and scheffe post-hoc test (p≤ 0.05).

  

  Results: the baseline hemoglobin, serum iron, ferritin, TIBC and TSAT in three groups were not different significantly. After intervention, the hemoglobin level of IVAA group( 20.1g/dl ) and in POAA group (0.99g/dl) were increased compared to control group(P>0.001). the mean of hemoglobin in IVAA was significantly more than POAA (P>0.05).

  

  Conclusion: based on the results, the use of oral and intravenous vitamin C can be effective in the anemia caused by iron deficiency, especially vitamin C injection that is more beneficial.

Abstract

  Background and Objective: nowadays, many menopausal women seek complementary and alternative drugs to relieve intensity of menopause symptoms. Most researchers are interested in studying medicinal plants containing isoflavones such as Red Clover with estrogen like properties. This study was conducted to determine the effect of red clover on the severity of physical menopause symptoms.

  Material and Methods: This randomized clinical trial was conducted on 72 menopausal women aged 50-59 in 2012. The participants of intervention group daily received two capsules containing 40 mg red clover for two months while the control ones received two capsules containing 40 mg starch in the same way. The data of baseline and the one of after two months were collected by demographic questionnaire and physical dimension of Menopausal Rating Scale and analyzed by SPSS version 14, using Man-Whitney and Student-t test.

  Results: Based on the mean and SD of physical menopause symptoms, the severity of flushing, nocturnal sweating and heart discomfort was reduced significantly compared to control group. Furthermore, all physical menopause symptoms were remarkably diminished (P<0.05).

  Conclusion: in accordance with the results, the capsule of red clover can be helpful in mitigating the severity of physical menopausal symptoms.

Abstract

Background and Objective: The people with Thalassemia, because of frequent blood transfusion and sever complication ,require constant medical supervision, and their psychological problems are paramount. This study aimed to determine Self-Concept and related factors in children and adolescents with Thalassemia major aged 7-18 in Golestan province ,2012. 
Material and Methods: This cross-sectional study was conducted on 155, using census sampling, children and adolescents with Thalassemia major refereeing to therapeutic and health centers for blood transfusion. The instruments were Piers-Harris self-concept questionnaire and demographic data . The analysis was performed by SPSS software using ANOV and t-test. 
Results: The mean of self-concept was low, and there was significant relationship between self-concept and variables such as age, educational status, dropout , order of birth, the number of children in family and parents’ education level. But no significant difference was observed between self-concept and the factors such as sport, the frequency of transfusion, economic status. 
Conclusion: The results revealed that the self-concept of the thalassaemic patients is low. Some non-adjustable factors exacerbate this condition, but encouraging them to continue their education, informing their parents more about their children’s disease can be helpful to improve their self-concept.

Abstract

Background: Chronic obstructive pulmonary disease (COPD) is a progressive, irreversible respiratory condition that imposes a significant physical and psychological burden, often leading to a poor quality of life. Although palliative care can help address these challenges, in Iran it is typically limited to cancer patients and is not commonly provided to those with COPD. Globally, the early integration of palliative care for chronic illnesses is expanding. The present study aims to investigate the feasibility and acceptability of an early tele-palliative care intervention for patients with COPD in Iran and to explore its potential effects on quality of life, anxiety, depression, and emergency department readmissions.
Methods: This randomized controlled feasibility trial protocol involves a 3-month early tele-palliative care program delivered by two nurse coaches, targeting patients with COPD. Participantswere randomly assigned to either the intervention group (n = 26) or the control group (n = 26) using permuted block randomization. Both groups received traditional COPD care; however, the intervention group also received six weekly telephone sessions and six weeks of follow-up support via phone call and WhatsApp Messenger. The primary objective was to assess the feasibility and acceptability of early tele-palliative care, measured through recruitment and attrition rates, questionnaire completion rates, patient satisfaction, attitudes toward the intervention, and adherence to the intervention. The secondary outcome included changes in quality of life, anxiety, depression, and hospital readmissions. These were measured using validated instruments at two time points: Baseline (pre-intervention) and three months post-intervention. Statistical analyses were performed using SPSS version 22, including the independent samples t-test, paired samples t-test, chi-square test, and Fisher’s exact test. Analysis of covariance (ANCOVA) was used to compare the mean outcomes between the two groups while statistically controlling for the effect of baseline values. Statistical significance was set at p < 0.05.
Conclusion: This study protocol aimed to determine the feasibility and acceptability of an early tele-palliative care intervention for patients with COPD in Iran. The findings provided essential data to inform the design of a future large-scale clinical trial and support the potential integration of early telehealth palliative care into standard COPD management in the country.
